Should you wish to utilize insurance, your insurance carrier may require a referral prior to your visit with us. This happens most frequently with HMO or other Managed Care plans. Our team would be happy to help answer questions regarding your particular carrier, work to obtain referral on your behalf, and answer any questions you may have.

Please be aware that while we are contracted with the carriers below, your particular plan may require authorization or referral prior to being seen. Our knowledgeable staff can answer any questions you may have regarding your specific insurance plan.

The cost of vascular treatment varies greatly depending on the procedure and the patient’s insurance coverage. Our team would be happy to provide you with an approximate cost of care for your treatment. We also offer payment plans and financial assistance for those who qualify.

No. Unless seen for an urgent on, as in the case of fistula declots, you will first be seen in office by one of our highly trained providers. During your new patient consult, your provider will partner with you to create an individualized plan of care for you specifically. Following your new consult, and should you have a plan for surgery, our surgery coordinators will contact you to schedule your procedure, go over pre and post procedure instructions, and discuss your cost of care.
