For your first visit, we ask that you arrive 15 minutes early and bring the following items:
- Insurance Card(s)
- Identification Card
- Method of Payment for Copay (if applicable)
- Medication List
- Filled Out New Patient Forms (download forms here)
During your consultation, your provider will explain and recommend an individualized plan of care for your condition.
